\r\nAttention: ALL 6th - 11th Graders
\r\nLHS students' activity athletic physicals will be administered on May 24th and 25th by Lee County Cooperative Clinic. Please complete the following forms. Transportation will be provided to the clinic and back to the campus. Forms can be downloaded (by clicking on the link below) or picked up at the Lee High School front office.
